All applications for the St. Luke’s Hospital / The University of Toledo Family Medicine Residency Program are taken through the Electronic Residency Application Service (ERAS). Paper applications are not accepted. Most candidates are selected through the National Resident Matching Program.

Applicants should submit the following documentation online through ERAS by December 31:

Our residency program does not have any absolute criteria about candidate USMLE scores; however, preference is given to candidates with scores above 220. Confirmation of passage of USMLE Step 2 is required before submission of our Match list. No one will be interviewed who has not taken Step 2, and any failures of Step 1 or Step 2 are viewed unfavorably.

International Applicants
We welcome international medical graduates to apply to our family medicine residency. We require a valid ECFMG certificate before we submit to our Match list. Our program will sponsor applicants with a J-1 or H-1b visa. Preference is given to international graduates who have meaningful United States (U.S.) or Canadian clinical training that involves patient decision-making.  U.S. experience in an observership role is more desirable than no U.S. experience at all.  We do not sponsor observerships.

Interview Selections
Interviews are by invitation and will be arranged upon receipt of all application materials. Preference is given to candidates who have graduated from medical school in the last three years. In addition, candidates who grew up or went to undergraduate or medical school in or around Ohio are viewed favorably. A demonstrated interest in family medicine also is preferred.

Candidates with prior postgraduate training in ACGME-approved programs will be considered for interviews. However, we do require details of that training, including specialty, duration, locations, program director, In-Training Examination results, and evaluations.

H-1b Visa Criteria/Guidelines

Residents may be eligible to enter the St. Luke’s Hospital / University of Toledo Family Medicine Residency Program on an H-1b visa under the following circumstances:

  1. Passage of USMLE Step 3 exam by date of rank order list certification.
  2. Scoring on USMLE Steps 1 and 2 greater than the mean.
  3. ECFMG certified at time of rank order list certification.
  4. Candidate must be considered highly desirable as evidenced by his/her level of clinical and/or research experience, scholarly activity, letters of recommendation, and quality of interview.
The University of Toledo through its affiliation with the St. Luke’s Hospital / University of Toledo Family Medicine Residency Program maintains the right to limit the number of H-1b visas offered based upon the criteria set forth by the institution/training program.

Residents requiring an H-1b visa will be responsible for the following initial visa expenses and renewal expenses:

  1. Lawyer/Immigration Specialist fees, as designated by the University of Toledo
  2. Premium processing fees (currently $1,500)
Residents requiring an H-1b visa will be required to use the lawyer/immigration specialist designated by the University of Toledo.

It is the H-1b visa holder’s (resident’s) personal responsibility to maintain lawful status while in the United States and under contract in the residency training program. Lawful status is evidenced by possession of a valid I-94 Record (Arrival/Departure Record) or I-797 Form (Notice of Action).
